Dementia Care Program

Our Dementia Care Specialty Program is inspired by the UCLA Alzheimer’s and Dementia Care (ADC) Program, a nationally recognized model for comprehensive dementia care.

This program is designed to address the complex medical, behavioral, and social needs of individuals living with Alzheimer’s disease and other types of dementia, as well as to support their families and caregivers.

Key Components of Our Program

  • Comprehensive Initial Assessment: A thorough evaluation by our Dementia Care Specialist to understand the patient’s medical history, cognitive function, and social environment.
  • Personalized Care Plans: Development of individualized care strategies that encompass medical treatment, behavioral interventions, and social support, tailored to the unique needs of each patient.
  • Ongoing Monitoring and Adjustments: Regular follow-up appointments to assess the effectiveness of the care plan and make necessary adjustments to ensure optimal care.
  • Caregiver Support: Access to our team for guidance and assistance, helping to prevent unnecessary hospitalizations and reduce caregiver stress.
  • Caregiver Education and Training: Provision of resources and training to caregivers, enhancing their ability to manage daily challenges and improve the quality of life for both patients and families.
